All fees including copays, deductibles, coinsurance, or out of pocket fees will be collected in full at the time of service. These are payable by cash, credit and/or debit card. Checks are NOT accepted. CNS Center of AZ accepts Visa, Mastercard, Discover, and American Express.

We aim to deliver care in a comprehensive, coordinated, and cost effective manner that meets the needs of patients, families, and the community.  Some forms of insurance are accepted for many of our services. A copy of your insurance card and driver’s license is required at the first visit. Those with out-of-network benefits are required to pay the full out of pocket cost for the visit. You as a patient or family member may submit to your insurance company yourself for reimbursement.
